She was predeceased by: her husband Warren L. Shively; her parents, John P. Mindock and Anna M. Klein (Duda); her granddaughter Theresa E. Shively; and her sister Helen M. Donchez.

She is survived by: her son David P. (Diane E.) of Orefield; and her grandchildren, Danielle Tackash, Valerie Shively, Kimberly Tackash, Christina M. Jiminez (Gabriel) and Kenneth Tackash. She is also survived by 5 great grandchildren.
