He is survived by: his wife Hope Early Forsythe; his son Michael S. Forsythe (Valerie) of Cincinnati, Ohio.; his grandchildren, Darren, Kyle, Freddy, Austin, Cody, Zachary, Kelsey, Peiton, Kaden, Brooklyn, Natalie and Amylya; his great grandchildren, Paisley, Lenny, Lyla and Wyatt; his sisters, Karen Schreckengost, Judy Myers, Fay Nelson, Dorothy Ferrari and Sharon Forsythe; and his son-in-law Frederick Wise Sr..

Visitation will be held on Wednesday, December 10th 2025 from 2:00 PM to 4:00 PM and from 6:00 PM to 7:00 PM at the H. Jack Buzard Funeral Home (201 S Washington St, Eau Claire, PA 16030).
