She was predeceased by : her parents, Ralph Andrews and Clara Andrews (Serfass); her husband William J. Kostak; and her sons, Todd Kostak and Franklin Andrews.

She is survived by : her children, William J. Kostak Jr. (Jill) of Jim Thorpe, PA, Denise Zellers of Jim Thorpe, PA, Michelle Roos (Frank) of Jim Thorpe, PA and Sherri Kostak of New Columbus, PA.; her grandchildren, Ashley, Jamie, Alisa, Tyler, Chad and Harley; and her great grandchildren, Lincoln, Myles and Riley. She is also survived by nieces & nephews.
