He was predeceased by: his parents, John Slifko and Margaret "Peg" Slifko (Krouse); and his brothers, Andrew and Robert Slifko. He is survived by: his wife Anna Marie "Annie" Slifko (Geiser) of Johnstown; his children, Amy Barkhimer (Gary) of Johnstown and Ray Slifko (Christina) of Bristol, PA.; and his grandchildren, Evan and Emily Barkhimer.
In lieu of flowers, donations may be made to the St. Jude Children's Hospital in memory of Jack Slifko.
