He was predeceased by: his father Dwight Jack; and his brothers, Craig and Eugene Jack. He is survived by: his mother Ruth Jack (Huss); his wife Susan Jack (Ickes); and his sisters, Debbie Marker and Sharon Gustine. He is also survived by many in-laws, nieces and nephews.
