He was predeceased by : his parents, Joseph R. Kwiatkowski, and Theresa Kwiatkowski, Sr. (Puller). He is survived by : his siblings, Theresa "Terry" Leskovec (Robert), Cathy Kwiatkowski and Robert Kwiatkowski (Susan); his nieces and nephews, Christine Kooser (Jason), Scott Leskovec, Matthew Leskovec, Andrew Kwiatkowski and Courtney Sattler (Ian); and his great-nieces and great-nephews, Madison, Jackson, Allison, Ariana and Natalia. He is also survived by many cousins and friends.
