He was predeceased by: his wife Anna E. Slaymaker (Martin); his parent John M.; his daughter Anna Mae Hertzler; and his grandchildren, Melody Rose Hertzler and Justin Slaymaker.
He is survived by: his parent Lizzy Slaymaker, Sr. (Eberly); his children, Dale Slaymaker, Marjorie of Washington Boro and John M. Slaymaker III; his spouse Martha Slaymaker-Walker of Columbia; and also Debi S. Slaymaker-Walker of Mount Joy. He is also survived by grandchildren and great-grandchildren.
