He is survived by: his spouse Jean Dzurek (Gallick); and his children, Nancy Worst (Daniel), David Dzurek (Kristine), Richard, Andrew Jr. Dzurek (Kimberly), Linda Ponegalek (Keith), Lorrie Maiolo (Frank), Brian Worst, Melissa Bosco, Jessica Dzurek, Megan Gatesman, Alicia Ponegalek, Amanda Orsi, Jennifer Maiolo and Frankie Maiolo.
In lieu of flowers, donations may be made to the Salvation Army or The National Kidney Foundation.
