He was predeceased by: his parents, Nicholas Ellsworth Fultz, Sr. and Grace E. Fultz (Specht); and his wife Evelyn Mae Fultz (Rhoades). He is survived by: his son Terry Lee Fultz of Milroy; and his sisters, Nancy Hannon (Gary) of Milroy and Betty Moore of Lewistown. He is also survived by six grandchildren; four great grandchildren.
