Foltz (Kenneth Sloat III) and Jason C. Foltz (Jennifer); his nephew Eugene Foltz; his brothers, Christopher C. Foltz and Benjamin J. Foltz; his aunts and uncles, Lynette Khanna (Naresh), Kenneth Ballou, H. Lee Ballou Jr. (Tracy), Janine Ballou and James Ballou; and his cousins, Olivia, Sarah Khanna, Nicholas, McKenna Ballou, Ashley and Julia Ballou.

In lieu of flowers, memorial contributions can be made to the American Foundation for Suicide Prevention (AFSP).
