He is survived by: his wife Josephine Pepera Ondulick; his daughter Kimberly; his son-in-law Clifford Gorski of Belle Vernon; his grandchild Marysa Jo Hazlett (Garrett) of Brandon Gorski; and his great grandchild Graham Hazlett.

The Family suggests memorial contributions in lieu of flowers be made in memory of Donald Ondulick to St. Sebastian Church.
