He is survived by: his wife Carol Surber Lewis; his sons, Jason Lewis (Adam) and Jeffrey Lewis (Mariann); his grandchildren, Michael Russell (Michaela), Jennifer Lewis and Zackary Lewis; and his friends, Bud, Bruce and Frank. He is also survived by three great-grandchildren; many beloved nieces and nephews; and many good friends.
