She was predeceased by: her husband John Dean Kushner; her parents, John M. Azzara and Lillian Azzara (Caputo); and her brother Louis John Azzara. She is survived by: her daughter Mary Patricia Petrarca (Domenic A.); her grandchildren, Brittany Lee Edmiston (Brett) and Domenic John Petrarca (Lauren); and her siblings, Joseph P. Azzara, Patricia L. Smith, Sharon E. Azzara and Sheila E. Morgan. She is also survived by many nieces and nephews.
