She was predeceased by: her parents, Frank Lewis and Anna Lewis; and her daughters, Janet Hackett and Phyllis Daino (Steve). She is survived by: her husband William I. Cook; her children, William L. Cook (Eileen), Sharon Guerra (Steve) and Nancy Lazarow (Keith); her grandchildren, Jackie (Dave), Steven (Jenny), Clint (Alycia), Michele (Jason), Christopher (Laura), Marikate (Jonathan), Christine (Tommy), Stephen, Maria, Ryan, Erica, Megan, Michael and Billy; her great grandchildren, David, Steven, Savannah, Bella, Emily, Cole, Livvy, Gabe, Harry, Claire and Willow; and her siblings, Robert Lewis (Kathy) and Arlene Collins (late Richard). She is also survived by many nieces and nephews.
