Kozlowski (Pittsburgh, Pennsylvania), born in Troy, Pennsylvania, who passed away on November 3, 2018, leaving to mourn family and friends. He was predeceased by: his grandparents, Kathy, Richard Krist and Florence Kozlowski. He is survived by: his parents, Deborah Krist of Shaler Twp and Jerome Kozlowski of Florida; his cousin Jordan Krist; his close friend Jaime Lunz; his pets, Capone and Camp Horne Road of Pittsburgh, PA.; and also Rick Krist.

In lieu of flowers, memorial donations in Jason's honor may be made to Animal Friends, 562 Camp Horne Road, Pittsburgh, PA.
