He is survived by: his father David; his son David Forrest III of Elyleria, OH.; his sisters, Lori August (Jim) of New Sewickley Twp and Tammy Romaniak (Bob) of Daugherty Twp; his nieces and nephews, Nate August (Natalie) and Madex; and also, Lyndsey August, Roxana Martenez and Areal.
