He was predeceased by: his parents, George Chuba and Susan Semancik Chuba; his wife Anne Flyak; his siblings, Anna, Mary Chuba, Sue Lysohir, Michael, John, George and Thomas Chuba; and his daughter-in-law Frances Chuba.
He is survived by: his children, William Chuba of Downingtown and Carol Walker (Scott) of New Cumberland; his siblings, Stephen (Joan) of Sharpsville and Helen Kachaylo of Brookfield, OH; and his granddaughters, Jean Chuba of Wayne, Sharon Garthwaite (Brian) of Lewisburg and Cathy Falci (Peter) of East Petersburg. He is also survived by 4 great-grandchildren; and numerous nieces and nephews.
