He was predeceased by: his father Edward Polowczuk; his son Steven E. Polowczuk, Jr.; and his brother Michael Polowczuk. He is survived by: his wife Anne V. Polowczuk (Bowe); his mother Doris Gilson Polowczuk; his daughters, Stephanie M. Polowczuk of Ephrata, PA, Sarah E. Polowczuk of Ephrata, PA and Samantha N. Polowczuk of Baltimore, MD; his stepson Brian E. O'Neill, Jr. of Palm Bay, FL; his granddaughter Madelyn J. Polowczuk of Ephrata, PA; and his brothers, Joseph "Joey" Polowczuk (Jill) of Lafayette Hill, PA and Timothy Polowczuk (Christa) of Douglassville, PA. He is also survived by many nieces and nephews.
