He is survived by: his children, Lynn Hayworth (Danny Nice) of Oakland, California and Lance Hayworth (Sharon) of Parrish, Florida; his stepchildren, Janice Lutton (Mark) of Pennsburg and David Schopfer (Trish) of Sarasota, Florida; and his brother Terry Hayworth. He is also survived by many grandchildren and great grandchildren.
