He is survived by: his sons, David Thomas (Stacie) of Waynesburg, PA and Brandon Thomas (Angel Burdine) of New Freeport, PA; his wife Debbie Lemasters; and his siblings, Donald Blake of Cameron, WV, Greg Blake (Rosanne) of Moundsville, WV, Ricky of Moundsville, WV, Harold "Bink" Blake (Mary) of Moundsville, WV, Nancy Lemasters of Moundsville, WV and Debbie Whitlatch (Don) of Graysville, PA.
