He was predeceased by: his parents, Rosalie Winnicki and Michael Douglas; and his wife Alberta (Forke). He is survived by: his children, Carol Kosik (Douglas), Thomas Douglas, Jr. (Gigi) and Robert J. Douglas (Aleydis); his grandsons, Nicholas (Allie), Matthew (Alli) and Jonathan; and his great grandchild Cade Michael Kosik.
