He was predeceased by: his wife Nancy E. Forsythe. He is survived by: his children, Mark Forsythe (Norma Jean) and Beth Layton; and his grandchildren, Christopher, Justin and Kyle. He is also survived by two great-grandchildren, a niece and a nephew.
