She was predeceased by : her spouse J. Walter Choquette, Jr.; her parents, John J. Nelson and Mattie E. Nelson (Betts); and her son David E. Choquette. She is survived by : her children, Leslie P. Choquette (Paul Kreswick) of Worcester and Keith L. Choquette (Beth) of Bristol.

Due to Covid restrictions, a Memorial Service for Phyllis will be held at a later date.

Friends who wish to do so may contribute in her memory to a charity of their choice.
