He was predeceased by: his parents, Henry E. Forrest and Frances M. Forrest (Percival); and his siblings, Phyllis St. Amand, Raymond Forrest and John Forrest.
He is survived by: his sister Barbara Geary of Cranston. He is also survived by many nieces and nephews.
