He was predeceased by: his spouse Stacia M. O'Hara (Wilczek); his parent Thomas F., Sr.; and his sibling Wilfred O'Hara. He is survived by: his parent Lavinia O'Hara (Caine); his children, David T. O'Hara of North Providence, Mary Ann O'Hara of Pawtucket and Bruce O'Hara; his sisters, Margaret Godlewski of Barrington and Maureen Buzzerio of California; his sister-in-law Alice O'Hara of Providence; his cousin John O'Hara of Narragansett; and his siblings, Joseph O'Hara and William. He is also survived by five grandchildren; six great-grandchildren.
