He was predeceased by: his parents, Von Church and Edna Church; and his siblings, Elizabeth, Victoria and David.

He is survived by: his wife Connie Church; his son Christopher Michael Church (Billie); his granddaughters, Sydney, Josie, Kennedy and London; his brother Audie Teran; his friends, George and Shirley Collert; and his daughter Sara Rebecca Ward.

Memorial contributions may be directed to the American Cancer Society.
