He is survived by: his father William Charles Chalk (Ruth) of Minneapolis, MN.; his daughters, Carrie Chalk of Inman and Kaitlin Chalk of Inman; and his sisters, Carrie Templin (Donald) of Finley, OH., Deanna Boyer of Crystal River, FL. and Kaye Czarnecki of Port St. He is also survived by 5 grandchildren.
