He was predeceased by: his father Grady Ezell Mullinax; and his brother Windel Earl Mullinax. He is survived by: his wife Tamara "Tammy" Mullinax; his children, Jon Michael Mullinax (Candice Ingram), Jodie Mullinax Foxx (Stevie Fox) and Shannon Mullinax McElroy; his grandmother Lou Fowler Mullinax; his siblings, Joey Mullinax (Teresa), Dean Mullinax (Judy), Charles "Bug" Mullinax, James "Monk" Mullinax (Dixie) and Brenda Mullinax Walker (Wayne); his grandchildren, Blake Mullinax, Jake Foxx, Dallas Sutton, Brooklyn Mullinax Huffstetler (Chris) and Bailey Sutton; and his great grandchildren, Ava-Grace Rae, Levi, Lane and Baby Huffstetler.
In lieu of flowers, memorials may be made in Johnny's name to Greene Funeral Home to help pay for his funeral.
