She was predeceased by : her husband Arnold; her parents, Martin and Mabel; and her siblings, Jim, Obed Osthus and Maurine Miller.

She is survived by : her sons, Mark, Paul and Philip; her sisters, Ardis Johnson of Beresford, SD and Carol Brende; and her sister-in-law Violet Osthus of Sioux Falls, SD.

Hazel's family requests memorials be directed to Scandia Lutheran Church or to Centerville Care & Rehab.
