He is survived by : his wife Nancy (Tisdall); his children, Chad Kenneth (Ambreen), Heidi Christine (Wolf), Andrea Dawn (Wolf), Diehl (Derek) and Pankoff (Jack Jr); his grandchildren, Alexa, Daniel, Eliya, Jack III, David, Taylor, Olivia, Logan and McKenzie; and his siblings, Don (Joanne), Cindy (Chuck), Patty (Jim) and Nate.
