He is survived by : his wife Terri Mayhew; his children, Leah Sullens (Johnny) and Brad Mayhew; his granddaughters, Grace Sullens and Brittney DeVincentis; his great grandchildren, Jordan Blackwell, Christopher Blackwell, MacKenzie Blackwell, Lucas Chastain and Addie DeVincentis; his great great grandchild Sloan Woeltje; and his sister Gail Cate.
