He was predeceased by : his parents, Hobert Leon Forrest and Mary Wheatley Forrest; his wife Charlotte Wheatley Forrest; and his siblings, Raydale Forrest and Sue Wright.

He is survived by : his daughter Amber Wells (Johnny) of Big Sandy, TN.; his brothers, Dwight Forrest of Camden, TN and Johnny Forrest of Big Sandy, TN.; his grandchildren, Dallas Evans (Danielle) and Evans of Montana; and his great grandchildren, Teagen, Jaxon and Emberly.
