He is survived by: his wife Lisa O'Dell Eleazer; his children, Shannon Eleazer, Bridgett Eleazer, Zack Eleazer, Drake Eleazer, Marcus Eleazer, Jacob Eleazer, Jamson Eleazer and Doug Eleazer; and his brothers, Jerry Eleazer, Jeff Eleazer, Jackie Eleazer and Jimmy Eleazer.
