He is survived by: his mother Mary Tilte Bloodworth of Hendersonville, Tennessee; his husband Jeremy Slade-Bloodworth of Clarksville; his sisters, Beth Holmes of Gallatin and Terri Bloodworth of Hendersonville; and his close friend Randy Talent of Kentucky.
