He was predeceased by: his parents, Creed Cook and Mary Brumett Bradshaw; and his siblings, Randy Cook, Ralph Cook and Carolyn Purdee.
He is survived by: his children, Gary Wayne Cook, II and Amy Michele Perkins (Rob); his grandchildren, Kyle Perkins, Alyssa Perkins and Piper Perkins; and his close friend Sharlene Brummett. He is also survived by several nieces, nephews and other family and friends.
