He was predeceased by: his parents, W. C. Cook and Mona Kay Waits Cook. He is survived by: his wife Lisa Cook; his children, Whitney Southall (Thomas) of Brighton and Zac Cook of Covington; and his sister Tina Anderson (Mike) of Covington.
