He is survived by: his mother Sandra Waynick Horn; his fiancée Angie Clark of Gallatin; his children, Logan Horn of Lafayette, Harley Horn (Brian Leveridge) of Lafayette, Lexie Allen (Britt) of Gallatin and Alyssa Clark (Ethan Brummett) of Bethpage; his sister DeeDee Masters (Steve) of Bethpage; and his grandchildren, Jase Leveridge, Jarrett Leveridge, Adalynn Allen, Palmer Allen, Indy Brummett and Kaden Brummett.
Interment will follow the service at Bethpage Upper Cemetery .In lieu of flowers, donations can be made to the funeral home to offset funeral expenses.
