He was predeceased by: his parents, Richard Cook and Eleanor Cook (Mitchell). He is survived by: his wife Connie S. Carter; his sons, Todd M. Cook of East Peoria and Ryan N. Cook of Frisco; his granddaughter Jennifer L. Cook of Portland, Oregon.; and his sister Janet Kaiser (Edward) of Peoria. He is also survived by several nephews and nieces.
