He was predeceased by: his parents, Aubrey Cook and Ruby Cook (Byrd); and his sister Sherry Lynn Shown. He is survived by: his wife Sandie Cook of LaFollette; his sons, Josh Cook (Lindsey) of LaFollette and Avery Cook (Kasha) of LaFollette; and his grandchildren, Tyler, Matthew Cook, Addison Cook and Adrienne King.
