He was predeceased by: his mother Denise Cook; and his grandfather James Cook. He is survived by: his father Jeff Cook (Constance); his sister Jensy Cook; his grandparents, Kenneth Cook (Lucille), Jean Cook and Wallace Phillips; and his stepsiblings, Robby Kelley, Timothy Kelley, Terry Wilson, Zach Wilson, Justin Wilson and Catie Moore.
