He was predeceased by: his father James Joseph Montague Jr.; and his stepfather Jim Jenkins. He is survived by: his mother Judy Brannen (John); his wife Daphne; his brothers, Matthew Montague (Casey) and Michael Jenkins (Katie Fernandez); his daughter Emilia Rose Montague; his mother-in-law Earlean Chenault; and his brothers-in-law, Ben Chenault and Jason Chenault. He is also survived by many loving family members and friends.
In lieu of flowers, the family requests that donations be made to the Juvenile Diabetes Research Foundation in honor of Jay's memory.
