He was predeceased by: his parents, Mack Cook and Dovie Cook. He is survived by: his siblings, Herbert, Clarence, Lowell, Leonard, James Russell, Carl, Ray, Fred Cook (Betty), Ella Boldin (Oliver), Bubbis Ruth Cook and Ann Widner (Randy). He is also survived by nieces, nephews, great nieces and nephews, cousins, other family, and friends.
