He was predeceased by: his parents, Bascom Sanford and Arline Sanford; his brother Geoffrey Sanford; his siblings-in-law, Holly Sanford, Dana Sanford and Stephen Polzer III; and his nephew Stephen Polzer IV.

He is survived by: his siblings, Cecelia Polzer and Michael Sanford. He is also survived by several nephews, nieces, great nieces, and great nephews and cousins.

In lieu of flowers, Jack requests donations be made to the St. Jude Children's Research Hospital or Shriners Hospitals for Children.
