He was predeceased by: his parents, Otto Ptacek and Lillie Ptacek; and his sister Frances Carnahan. He is survived by: his wife Karen Brown Ptacek; his daughter Jennifer; his son-in-law Brandon Moon; his son Anthony; his daughter-in-law Hannah Ptacek; his grandchildren, Cooper Moon, Payton Ptacek, Preston Moon and Logan Ptacek; his sister Alice; and his brother-in-law Eddie Joe Orsak. He is also survived by nieces, nephews.
Visitation was held on Thursday, December 5th 2024 from 6:00 PM to 8:00 PM at the Greenwood's Live Oak Chapel (3100 White Settlement Rd, Fort Worth, TX 76107). A funeral Mass was held on Friday, December 6th 2024 at 10:30 AM at the Holy Family Catholic Church (6150 Pershing Ave, Fort Worth, TX 76107). A graveside service was held on Wednesday, December 11th 2024 at 10:15 AM at the Dallas-Fort Worth National Cemetery (2000 Mountain Creek Pkwy, Dallas, TX 75211).
In lieu of flowers, the family requests a donation be made in Nick's name to the Ronald McDonald House in Fort Worth, or to Texas Parks and Wildlife.
