He is survived by: his spouse Gwenda Cook (Gwen) of Lufkin; and his children, Susan Elizabeth and John David Sullivan. He is also survived by his wife and two children.

In lieu of flowers, the family request memorials to Hill Country Memorial Hospital, Hill Country Memorial Hospice, the Pioneer Memorial Library of Fredericksburg or the donor's favorite charity.
