He was predeceased by : his parents, Joe Cook and Elsie Cook; and his siblings, JT Cook, Nina Combest and Ethelyn Byrd (Scooter).

He is survived by : his spouse Esta Lee "Scooter"; his children, Donna Querner (David) and David Cook (Melissa); his grandchildren, Tyler (Samantha Lewellen), Tara Hanks, Lanese Barnett (Brady), Zach and Caleb Cook; and his great grandchildren, Westin, Grayson Hanks, McKinna, Madelyn, Mia Lewellen, Talia and Jolie Barnett.
