He is survived by: his wife Oleta Anderson Chiavacci of Phoenix, Arizona.; his children, Cathy Byrd (Jeff), Cindy Shaw (Galen), Richard Chiavacci (Christy) and Anthony Chiavacci (Mary); his sister Marie Krull (Joe); his grandchildren, Lindsey Shaw, Ryan, Scott Byrd, Brandon, Bryan Chiavacci, Brady Parker and Kiley Hill; his great grandchildren, Paisley, Bryley, Dalton Vackar, T. J., Matthew, Karina Byrd, Alize Chiavacci, Hope and Gabby Hill; and his caregivers, Valerie Thomas and Melinda Reyes.
