He was predeceased by : his parents, Thelma Smith and Hazen Thomas Smith Sr. Hazen; and his daughter Denise. He is survived by : his wife Tracy Michelle Kisor; his sister Sherry; and his sons, Tommy (Deanna), Parris and Troy (Jennifer). He is also survived by nine grandchildren and seventeen great grandchildren.
