He was predeceased by: his parents, John Slovak and Annie Slovak; his wife Anita Cartwright Slovak; and his brothers, Richard, Daniel, Sydney and BJ Slovak.
He is survived by: his children, David Slovak and Jonna Strantz (Jon Strantz); and his grandchildren, Lance, Keith Klepper, Kregg Slovak, Maggie Weber, Rachel Slovak, Meggan St Nicholas and Erin Larson. He is also survived by six great-grandchildren.
