He was predeceased by : his wife Geanne; and his sister Dixie Park. He is survived by : his sons, Jeff Forsythe (Sara) of Broken Arrow and Jay Forsythe of Tahoka; his brother Larry Forsythe (Nancy) of Lubbock; his grandchildren, Kalyn (Hayden), Kody, Josh, Erin, Sam, Justin, McKenzy (Josh), Jaxon and Bretton; and his great grandchildren, Covi and Casey.
